Pre-eruptive caries
#20 gone but not forgotten.
CC: “My daughter’s tooth is dark and sensitive.”
Dx: #20 pre-eruptive intra-coronal resorption with irreversible pulpitis
Tx: extraction
🔑: Did you know teeth can get “cavities” before they even erupt into the oral cavity? This is the most extreme case of pre-eruptive intra-coronal resorption (aka pre-eruptive caries) where there’s resorption of the dentin extends from the DEJ to the CEJ. Unfortunately, with the pulpal involvement, this tooth was deemed nonrestorable and extracted. The crown of the tooth immediately separated from the root upon elevation of the tooth and let’s just say it did not smell good when that happened. 🤢
So what is PEIR? The literature is pretty sparse but histological studies show there’s involvement of multinucleated giant cells, osteoclasts, and chronic inflammatory cells leading to resorption of the hard tissue.
